CLAIMS

Claims:

  • The funeral benefit covers the maximum of 1 spouse and 5 children

  • A stillborn is regarded as a child that has had at least 26 weeks of intra-uterine existence but showed no sign of life after complete birth

  • The nomination forms merely serves as a guide to the trustees in identifying persons who could potentially qualify as dependants

Withdrawals:

  • Ensure that the members complete and sign their Withdrawal forms

  • Provide us with the members contact details Members Personal Income reference number must be completed

  • Ensure that we received the following 3 documents

  • Completed forms with the members signature and reason for withdrawal

  • Clear ID copies    

  • First page of a current Bank Statement with the bank stamp whereby the name of the account holder and account number is legible

  • Members personal income tax reference number must be completed

  • Members contact number/s

Notification Periods:

  • Funeral claims must be submitted to the Fund ( 3 months from date of death )

  • Death claims must be submitted to the Fund ( 6 months from date of death )

  • Disability claims must be submitted to the Fund ( 3 months from date of accident, injury or diagnosis )

Please be advised that late notification may be grounds for insurers repudiation of a claim
